  • All calls returned within 24 hours
  • VHCS provides advocacy (liaison) and education on health related topics

Employer Plan- “24/7 phone a nurse with questions”
  • This service is paid by the employer
  • Special rate is available to the employer if all insured employees are provided the service on an annual basis
  • All services outlined above under Included Services are included


Individual Annual Plan- “24/7 phone a nurse with questions”
  • This service is paid by an individual
  • Annual plan is $499.00
  • All services outlined above under Included Services are included


Transition of Care- “I want to go home”

Vic’s Healthcare Concierge Services will customize a package to meet the following needs. If your need is not listed, give us a call and we will try our best to accomodate your request. Our mission is to advocate and educate healthcare consumers while coordinating community resources to meet your needs.

Basic Transition Care Package $850.00 (actual mileage will be billed separate)
Services included: Please purchase package 48 hours prior to anticipated need

  1. Six hours of a Care provider at time of discharge (on day of discharge)
    1. present to discuss discharge instructions prior to leaving the facility
    2. transport you home
    3. shopping as requested (pharmacy, food, etc.)
    4. get you settled at home (basic house cleaning, laundry, meal, review discharge instructions for clarity)
    5. assist to access hospital record via patient portal (instructions from hospital)
    6. available to interface with any other services in the home to ensure coordination of care to maximize outcomes
  2. One day a week for 4 hours to visit for 6 additional weeks post discharge to assist with chores under your direction (clean, laundry, prep meals, shop, personal care, assist to make appointments, etc.)
  3. Phone access to a Registered Nurse 24/7 for 45 days after discharge
    1. Call (989) 205-0511 anytime, as often as you need with any healthcare related question. Phone conversation for the purpose of education, advocacy will be provided
      1. Explain disease process along with specific care related
      2. Review meds and purpose, side effects
      3. Coordinate doctor instructions


ACO's & Physician Plan- “Educate my patients and their families”
Vic’s Healthcare Concierge Services is a fee for service business and customizable to your needs. Call us and schedule a time to discuss your specific needs. Including chronic care management.
  • This service is paid for by the physician or ACO
  • This service is provided to physicians to assist with:
    • Patient education to enhance patient experience
    • To improve population health outcomes
    • Education of patients without family support
    • Patients that visit frequently due to lack of compliance
    • High profile patients that you “want” to go the extra mile
    • Pre-op through Post-Op to ensure best outcomes for “challenging situations”
  • Cost is negotiated according to the number of patients and services being requested


Hospital Plan- “Help I need patient engagement Now!”
Vic’s Healthcare Concierge Services is a fee for service business and customizable to your needs. Call us and discuss.
  • This service is paid for by the hospital
  • Service is available on case by case situation or monthly, annual contracts (much less than penalties)
  • Service is designed to assist the hospital with
    • Patients that impact readmit rates
    • Patient satisfaction scores
    • Patients without family support
    • Patients with multiple disease processes
    • High Profile Patients
  • Staff education to enhance the patient experience and improve patient outcomes
    • In-service options and customizable delivery models available


Homecare Agencies & Long Term Insurance
  • This service is paid for by the homecare company or long term insurance
  • Call us to discuss options for working with "chronic care management" needs
  • Staff Education, Patient Education, Family Education
  • Our services offset the cost of extended skilled visits and penalties for reopens
  • Use long term insurance to pay for homecare needs.


Agent Plan- “I want to provide top services to my clients”
Vic’s Healthcare Concierge Services is a fee for service business. VHCS welcomes insurance agents, financial planners, family-life planners or any others that wish to make our services known and contracted by any of their clients. VHCS will pay a fee on all business that is written for VHCS by third party “agents”.
  • Agents may consider the following rational for including VHCS as an offering to their clients:
    • VHCS helps to promote free preventative care that is part of health insurance policies, and may help to drive lower insurance premiums (good for employers)
    • VHCS help clients stay healthy, which means clients remain independent and home
    • VHCS helps clients manage chronic disease process, so they can remain independent and home
    • VHCS helps clients improve healthy behaviors, improve their health and remain home
